Dermal fillers are non-surgical injectable treatments used to restore lost facial volume, soften lines and improve facial contours. When used carefully and conservatively, they can produce subtle changes that help you look fresher without looking obviously “treated”.
Our approach begins with careful assessment of your facial anatomy, how your face has changed with age and, most importantly, what you would like to achieve.
What Are Dermal Fillers?
Most modern dermal fillers are soft injectable gels containing hyaluronic acid, a substance that occurs naturally in the skin and throughout the body.
When carefully placed beneath the skin, hyaluronic acid fillers can replace lost volume, provide support to facial tissues and improve contour and proportion as well as improving surface lines..
Different fillers have different characteristics, and the choice of product, amount and placement should depend upon the area being treated and the result you are trying to achieve.

Why Does Our Face Lose Volume as We Age?
Facial ageing is not simply about developing lines and wrinkles.
Over time there are changes in the skin, facial fat, supporting tissues and underlying bone. These can contribute to loss of facial volume and support, creating hollows, folds and changes in facial proportion.
Dermal fillers cannot reverse the ageing process, but carefully placed filler can restore support or volume in selected areas where these changes have occurred.

What Can Dermal Fillers Treat?
Dermal fillers can be used to address a range of concerns, including:

Not every area is suitable for every patient, and sometimes filler is not the most appropriate treatment at all.
That is one reason why assessment before treatment matters.
Who Is Suitable for Dermal Fillers?
Dermal fillers may be appropriate for adults who have a specific area of volume loss, structural change or facial imbalance that can reasonably be improved with treatment.
Suitability depends upon your facial anatomy, skin and tissue quality, medical history, medications, previous treatments and expectations.
There are also circumstances in which treatment may need to be postponed or avoided, including certain medical conditions, active infection or inflammation in the proposed treatment area, pregnancy or breastfeeding, and some medications or allergies.

At your consultation in our Esher clinic we will:

Sometimes the outcome of a consultation is that we recommend a different treatment or occasionally no treatment at all.
Our aim is to provide clarity, not pressure.
What Happens During Dermal Filler Treatment?
Before treatment, the proposed areas and treatment plan will be explained and discussed again. There will be plenty of time to answer any further questions.
Depending upon the area being treated, filler will be placed using a fine needle or micro-cannula.
The amount of filler required varies considerably. Our preference is to use the amount needed to achieve the intended result rather than treating to a predetermined quantity.
The actual treatment itself is usually relatively quick, although we allow sufficient appointment time for assessment, preparation and careful placement rather than rushing the procedure. At the Cosmetic Doctors Company our appointments are never rushed.
Does Dermal Filler Treatment Hurt?
The majority of our patients find dermal filler treatment very manageable.
We use topical anaesthetic cream to make treatment more comfortable and, for certain procedures, small amounts of injectable local anaesthetic may also be used. The Belotero brand of hyaluronic acid fillers we use also contain lidocaine, a local anaesthetic.
You may feel pressure, pinching or brief discomfort during injection. This varies according to the area being treated and individual sensitivity.
The combination of topical anaesthetic, local anaesthetic where appropriate, and the lidocaine contained within the filler helps to minimise discomfort.
When Will I See My Result?
Unlike anti-wrinkle injections and treatments that depend upon collagen stimulation, dermal fillers produce an immediately noticeable effect.
However, what you see immediately after treatment is not necessarily the final result. Some swelling is common and can temporarily exaggerate the effect of the filler.
As swelling subsides and the filler settles into the tissues, the result becomes easier to assess. For most treatments this takes around one to two weeks.

Natural Results — Refinement, Not Transformation
For us, the goal of dermal filler treatment is refinement, not transformation.
A well-performed treatment should respect your existing facial proportions and should not announce itself as filler.
Patients frequently tell us that their greatest concern is looking overfilled or unnatural. That is entirely understandable. The problem is rarely the existence of dermal filler itself; it is more often inappropriate patient selection, poor treatment planning or simply using too much.
Our preference is for conservative treatment that restores what has been lost or improves a specific area without changing the fundamental character of the face.

How Long Do Dermal Fillers Last?
Dermal fillers are temporary treatments.
How long they last varies according to the filler used, where it is placed, the amount used and individual biological factors. In many facial areas, results may last somewhere between approximately 6 and 18 months, although some treatments may last for a shorter or longer period.
Importantly, treatment does not suddenly disappear on a particular date. Hyaluronic acid filler is gradually broken down by the body and the effect diminishes over time.

Are Dermal Fillers Safe?
Dermal fillers have a well-established safety record in aesthetic medicine, but they are medical treatments and they are not risk-free.
Common temporary effects include swelling, tenderness, redness and bruising around the injection sites.
There are also uncommon but potentially serious complications, including vascular occlusion, where filler compromises the blood supply to tissue.
This is one reason why knowledge of facial anatomy, careful injection technique and the ability to recognise and manage complications are important.

Can Dermal Fillers Be Dissolved?
One advantage of hyaluronic acid dermal fillers is that they can, when clinically appropriate, be dissolved using an enzyme called hyaluronidase.
This may be required to manage certain complications or occasionally where a filler result needs to be reversed.
The fact that hyaluronic acid filler can be dissolved is reassuring, but it should not be regarded as a substitute for careful assessment, appropriate product selection and good injection technique in the first place.

Is There Any Downtime?
Most patients return to their usual activities shortly after treatment.
Some swelling, redness, tenderness or bruising is common, however, and this can be more noticeable in certain areas — particularly the lips.
It is therefore sensible not to schedule dermal filler treatment immediately before an important social event.
After Your Dermal Filler Treatment
We will explain any specific aftercare relevant to your treatment before you leave the clinic.
In general, it is sensible to avoid unnecessarily touching or pressing the treated area immediately afterwards and to avoid strenuous exercise and excessive heat for a short period where advised.
Most importantly, if you have any concerns following treatment, you can telephone us for advice or arrange to be seen earlier.
Your care does not end when you leave the treatment room.
Follow-Up and Maintenance
We normally suggest a review approximately 3–4 weeks after dermal filler treatment.
This allows time for any initial swelling to settle and gives us an opportunity to assess the result properly.
There is no need to wait for the routine review if you have any concerns. You are always welcome to contact us or arrange to be seen earlier.
Over time, hyaluronic acid filler is gradually broken down. Some patients choose to repeat treatment as its effect diminishes, while others are happy to allow it to disappear.
There is no requirement to continue treatment.
How Much Do Dermal Fillers Cost?
Our current dermal filler prices are:
At your treatment appointment
1.0ml syringe — £425
Second and subsequent 1.0ml syringes at the same visit — £300 each
0.6ml syringe — £375 when used as the only syringe
Second 0.6ml syringe at the same visit — £250
Tear trough treatment, 1.0ml or 1.2ml — £495
Where further filler forms part of the same treatment plan and is used at a follow-up appointment within 4–6 weeks:
1.0ml syringe — £325 each
0.6ml syringe — £275 each
The amount of filler required depends upon the area being treated and what we are trying to achieve.
We will discuss the proposed treatment, likely amount of filler and cost with you before you decide whether to proceed.
