Every few years, a new treatment promises to transform aesthetic medicine. Some genuinely change clinical practice. Others quietly find a more limited role. Why does this happen so often? Dr Peter Forrester explores the surprisingly familiar life cycle of medical innovation.

It starts on Instagram. A celebrity mentions a treatment, an influencer documents their experience, a hashtag accumulates momentum. Within weeks, clinics are adding it to their menus and patients are asking for it by name. Sometimes the enthusiasm is warranted. More often, the excitement has considerably outrun the evidence. We have watched this cycle operate for over twenty-five years. And we do not follow it

General practice is a discipline of breadth — producing doctors who are comfortable with uncertainty, skilled at listening, and trained to consider the whole patient rather than the presenting complaint. These turn out to be exactly the skills that distinguish good aesthetic practice from merely competent treatment delivery.

Most patients who consider aesthetic treatment have a sense that looking better might make them feel better. It is an instinct the aesthetic industry is happy to exploit and that medicine has been reluctant to validate. The honest clinical answer sits somewhere between the two — and it is worth stating clearly.

For many women, looking well is not simply a personal preference. They are in roles where their appearance is professionally visible — and the pressure to maintain a youthful appearance in professional life falls disproportionately and significantly on women. This piece is for them.

When a woman presents for an aesthetic consultation shortly after a divorce, there is an assumption that floats in the background. She wants to look her best for a new audience.

In our experience, this assumption is almost always wrong — and what these patients are actually seeking is considerably more interesting and considerably more human.

Ask most patients what they are hoping for from aesthetic treatment and the answer comes down to the same thing. They want to still look like themselves. It is the part that matters most — and yet it is the hardest to define, hardest to measure, and hardest to consistently deliver.

The aesthetic industry owes a genuine debt to celebrity culture for the normalisation of treatment and the removal of stigma. The acknowledgement should come with a significant caveat. The relationship has never been straightforward — and its influence on patient expectations has been considerable and largely negative.

Most men who come to us for a first aesthetic consultation have been thinking about it for some time. Not days or weeks. Often months, sometimes years. They have noticed something changing in the mirror and have been sitting with it quietly, wondering whether anything can be done — and whether doing something about it is something they are prepared to do.
